Becoming a Hospice Volunteer

We are so excited that you are interested in volunteering at St. Luke’s Hospice. We believe you will truly make a difference in someone’s life through your efforts.

Being a Hospice volunteer is definitely a commitment and we appreciate your willingness to give of your time and efforts. As a Medicare-funded agency, the government requires that all St. Luke’s Hospice volunteers complete all the paperwork provided below.

Once all the forms have been completed, please mail the entire packet back to us and we will contact you for an interview. An interview precedes all training and does not necessarily ensure you will become a St. Luke’s Hospice volunteer.

  2. Three (3) written references (download here)
  3. A photocopy of your current driver’s license and auto insurance card
  4. Job description (download here)
  5. Criminal Record Release Forms
    1. State of Iowa Background Check
    2. National Background Check
    3. DHS Form

Once received, the Volunteer Coordinator will contact you to schedule a face to face interview.

 

The items listed below will be provided to you following your interview and will need to be completed prior to training:

  1. Physical Assessment and Completed 2-step TB testing (provided by St. Luke’s Employee Health)
  2. Name Badge procurement
  3. Volunteering Information Survey

 

All volunteers wishing to work directly with patients will need to complete a 14 hour training held seasonally at St. Luke’s Hospice before visiting patients.
