Lung Care Coordinator
Lung Care Coordinator -- is a registered nurse with oncology experience that acts as a single point of contact for doctors and patients. The coordinator’s mission is to help patients and families get the care they need by:
• Working as a partner each patient's doctor and keeping the doctor informed of patient’s care plan.
• Providing patients with a complete work-up based on evidence-based guidelines and the patient's family doctor's preference.
• Providing patients immediate access and coordination of necessary services for detection, diagnosis and treatment.
• Ensuring specialist recommendations flow efficiently back to the patient and family doctor in an organized, timely manner.
